N.B. If you request the destruction of your medical file, the clinic will determine whether we destroy your data.
If your data is deleted, your written request will be retained. Together with the letter that your request has been met. If there is a need for information from your ‘old’ file during subsequent treatment, it will only be found that it has been destroyed at your request. As a patient, you may break the continuity in the treatment data. Please note that you are destroying your history of illness. This can be difficult for future treatments. Your file will be destroyed within 3 months of your request.
A practitioner may have medical reasons for not complying with your request to delete data from your medical file. For example, if the destruction would hinder the practitioner’s proper care. Or to prevent a particular investigation from having to be conducted again. Data that may be of great importance to others – for example about a hereditary disease – will also have to be kept. If we decide not to delete your data, we will explain this to you with substantiation.
Have you started a complaints procedure as a result of treatment? Then we will not destroy your file. After all, the file contains information about your treatment.
